This one is a bit tricky on a global scale, I would imagine, as I'm sure everybody has differing opinions on the matter. As it stands at the time of writing, it looks like people are gravitating towards the middle two options.
Personally, I'm torn whether to vote for all options or none at all, as my opinion on the matter is more or less "whatever happens, happens". That being said, particularly for the last option, I wouldn't *particularly* like to "see the chance of permanent character death", but if it happens, I absolutely wouldn't argue provided the reasoning behind such a decision is sound.
Angel is perhaps a good example here. Went into a suspected enemy stronghold knowing the risks and literally put her life on the line in an attempt to save a prisoner. My rolls were abysmal and I absolutely expected her to die fighting.
Perhaps a good way to approach something like permanent death is to let the player know that it could happen, if the player so chooses to pursue their actions regardless of warning(s).
